Advance Care Planning is a conversation and documentation process that ensures your healthcare wishes are clearly recorded and respected if you’re ever unable to speak for yourself. We help you explore values, goals and treatment preferences—and put them into an easily accessible plan.
Why It Matters
Clarity for Whānau & Clinicians Everyone knows your wishes—reducing stress and uncertainty in difficult moments.
Empowerment You remain in control of your care, even if you can’t communicate later.
Stronger Connections Honest conversations can bring family and friends closer, aligning everyone around what matters most to you.
What We Offer
Guided Conversations A trained advisor meets with you (and whānau, if you choose) to discuss values, fears and treatment options.
Documentation Support We walk you step-by-step through Advance Directives, Enduring Power of Attorney forms and any legal paperwork.
Review & Update Your plan stays current—annual check-ins or as life circumstances change.
Resource Materials Booklets, worksheets and online tools to help you reflect, prepare and share your plan.
