    Advance Care Planning is a conversation and documentation process that ensures your healthcare wishes are clearly recorded and respected if you’re ever unable to speak for yourself. We help you explore values, goals and treatment preferences—and put them into an easily accessible plan.

    Advance Care Planning is available to any adult living in Mid-Northland who wishes to plan ahead, as well as families supporting a loved one’s healthcare choices and health professionals seeking guidance on ACP for their patients.

    Distribute copies to your GP, whānau and nominated attorney. We’ll check in annually or whenever you wish to update.
