    Referrals & Access

    Q: How do I refer myself or a loved one to Hospice Mid-Northland?
    You can self-refer by calling (09) 407 7799 or by completing our online referral form at /referral-form. Alternatively, your GP, district nurse or specialist can refer you on your behalf.

    Q: What happens after I submit a referral?
    Within 24 hours we’ll acknowledge receipt. A specialist nurse will then contact you—usually within 48 hours—to arrange an initial assessment and co-develop a personalised care plan.

    Q: Who is eligible for hospice services?
    Any Mid-Northland resident with a life-limiting illness can access our palliative care, bereavement support and advance care planning services—regardless of age, diagnosis or ability to pay.


    Services & Funding

    Q: Is there any cost for hospice care?
    No. All clinical visits, counselling and advance care planning services are provided free of charge, thanks to our mixed funding model (37% government, 31% retail revenue, 32% community fundraising).

    Q: Which services do you offer?

    • Home Palliative Care: Symptom management and emotional support in your home.

    • Bereavement Support: One-to-one counselling, group sessions and printed resources.

    • Advance Care Planning: Guided conversations and documentation of your healthcare wishes.

    Patient & Whānau Resources

    Q: Where can I download guides and fact sheets?
    Visit our Resources page for free PDFs on palliative care, symptom management, advance care planning and grief support. Hard copies are available on request.

    Q: How do I join a grief support group?
    Email hello@hospicemn.org.nz or call (09) 407 7799 and ask for Bereavement Support. We’ll provide upcoming group session dates and venue details.


    Volunteering & Donations

    Q: How can I volunteer with Hospice Mid-Northland?
    We welcome volunteers for retail, sorting donations, events and administrative support. Apply online at /volunteer or email volunteer@hospicemn.org.nz for an application form.

    Q: What goods can I donate, and where do I drop them off?
    We accept quality second-hand clothing, furniture, homewares and books. See our Donate Goods page for a full list and drop-off locations.

    Q: How do I make a financial donation?
    Give online at /donate, shop in one of our op-shops, or leave a bequest. Every dollar raised is spent locally to support Mid-Northland families.


    Health Professional Enquiries

    Q: I’m a GP—how do I refer my patient?
    Complete our secure e-referral form at /referral-form or call our Intake Team on (09) 407 7799 to discuss clinical suitability.

    Q: Can I arrange a team debrief or grief-awareness training for my staff?
    Yes—contact our Bereavement Support Coordinator at hello@hospicemn.org.nz to schedule in-service training or group sessions.


    After-Hours & Urgent Support

    Q: What do I do if urgent symptoms arise outside office hours?
    Call our 24/7 on-call number: 0800 HOSPICE (0800 467 742). A nurse will triage your call and arrange advice or a visit if needed.

    Q: Who do I contact for equipment breakdowns or emergencies?
    For urgent equipment issues, call our Equipment Coordinator on (09) 407 7799 and follow the prompts for “equipment support.”
