    What Is Community Palliative Care?

    Our Community Palliative Care service delivers specialist nursing and allied-health support directly to you—wherever you call home. We focus on:

    • Pain & Symptom Management: Expert assessment and medication reviews to keep you as comfortable as possible.
    • Care Coordination: Liaising with your GP or NP, specialists and community services to suport seamless care.
    • Emotional & Psychosocial Support: we provide counselling, family &  whānau support and liaise with other services including grief support.
    • Practical Assistance: Coordinating with other services like home support, medication reviews, equipment and respite.

    Who Can Access This Service?

    • Residents of Mid-Northland with a life-limiting illness
    • People in private homes, rest homes or supported-care facilities
    • Self-referrals welcome; however referrals via a health care professionals preferred .
