    Referral Process

    Connecting you with the care you need—simply and compassionately.

    At Hospice Mid-Northland, we aim to make referrals seamless for patients, whānau and health professionals. Our Referral Process ensures timely access to palliative care, bereavement support and advance care planning services—completely free of charge.

    Who Can Refer

    • Health Professionals: GPs, district nurses, specialists and allied-health clinicians can submit a referral on behalf of their patient.

    • Self-Referral: Individuals and whānau are welcome to refer themselves by contacting our Intake Team directly.


    Referral Methods

    Online Form
    Complete our secure e-referral at hospicemn.org.nz/referral-form—available 24/7.

    Phone
    Call us on (09) 407 7799 and our Intake Coordinator will guide you through the questions.

    In Person
    Pick up or drop off a paper form at:

    • Hospice Mid-Northland Office, 464 Kerikeri Road, Kerikeri

    • Any Hospice Mid-Northland retail shop


    Information to Provide

    To help us deliver the right care plan, please include:

    • Patient’s full name, date of birth and contact details

    • Referrer’s name, role and contact information

    • Current diagnosis and prognosis (as known)

    • Key concerns (pain, symptoms, emotional needs)

    • Preferred location of care (home, rest home, day-therapy)

    • Any cultural, spiritual or language requirements

    What Happens Next

    1. Acknowledgement: We aim to confirm receipt of your referral within 24 hours.

    2. Triage & Allocation: Our clinical team reviews each referral to determine urgency and service type.

    3. Initial Assessment: A specialist nurse will contact the patient (or family) to arrange a home or facility visit—usually within 48 hours of triage.

    4. Care Plan Development: Following assessment, we co-create a personalised plan outlining services, visit frequency and support contacts.


    Confidentiality & Consent

    All information you provide is treated as strictly confidential under New Zealand health privacy laws. We will seek informed consent from the patient or their authorised decision-maker before sharing details with other healthcare providers.
